When I first started at my job back in 2014, we were still a completely paper chart facility. All medication and treatments were ordered on paper and automated by pharmacy once a week. If there were changes made to any order after Thursday afternoon (for the coming week), they had to be made by hand by the nurse because pharmacy printed the MARs in advance. This process was not only time consuming, but also confusing for a new grad like myself. I would have to discontinue on the current MAR, find the new MAR and discontinue there and then transcribe the new order on both. To me, that was just a disaster waiting to happen.
